Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, well-being, and identifying care barriers. Perform medication and medical history reviews alongside physical evaluations using pre-populated, patient-specific forms. Manage visits with health plan members in their homes to educate them on treatment plans and empower them as health advocates. Work Monday through Sunday with flexible scheduling, averaging 6-9 visits daily at a base rate of $80 per assessment. Earn supplemental compensation via a pay-per-test model and market multipliers. Requires a state license or willingness to enroll for Medicaid. Independent contractor status with malpractice coverage and travel support available.
