Job Summary
Conduct in-home visits for Medicare Advantage, Medicaid, and commercial plan members to perform medication and medical history reviews, physical evaluations, and identify care barriers. Complete assessments using pre-populated, patient-specific forms without prescribing medication, ordering lab tests, or altering treatment regimens. Manage an average of 6-9 daily visits with flexible scheduling from Monday through Sunday, earning up to $4,000 weekly via a base rate and market multiplier. Work as an independent contractor with available travel support including airfare and hotel.
Required Qualifications

- Medicaid License
- State License
- Independent Contractor, 1099 status
