Nurse Practitioner for In-home Visits

Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members using provided patient-specific forms without prescribing medicine, ordering lab tests, or altering treatment regimens. Complete an average of 6-9 visits daily to identify barriers to care and support patient re-engagement. Earn weekly compensation ranging from $2,500 to $4,000, including a base rate of $80 per assessment and potential pay-per-test bonuses. Work flexible shifts Monday through Sunday with incentives for availability. This role is available for short-term or long-term basis as an independent contractor.
