Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members by reviewing medication history, conducting physical assessments, and identifying care barriers. Perform pre-scheduled visits with health plan members using provided patient-specific forms while managing medication reviews and medical history documentation. Earn up to $4,000 weekly with an average of 6-9 daily visits, utilizing a pay-per-test model for additional diagnostic and preventive services. Work as an independent contractor on short-term or long-term bases with flexible Monday through Sunday availability.
