Job Summary
Board-certified Nurse Practitioner contractors needed to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Conduct in-home visits focusing on overall health and barriers to care, with pre-scheduled visits in members’ homes. Earn a competitive weekly range ($2,500–$4,000) with base rate of $80 per completed assessment and potential increases via a Market Multiplier; additional pay via pay-per-test model for qualifying diagnostic/preventive tests. Flexible schedule with shifts Monday–Sunday (no nights/weekends). Licensure required (TN) with Medicaid license enrollment supported. Independent contractor status (1099) with malpractice coverage and airfare/hotel/rental car availability depending on eligibility. Location: Springfield, TN (US).
