Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members by reviewing medication history, assessing physical condition, and identifying care barriers. Perform pre-scheduled visits with health plan members using provided patient-specific forms to educate them on treatment plans without prescribing, ordering labs, or altering regimens. Complete an average of 6-9 daily assessments across Monday through Sunday shifts, earning up to $4,000 weekly via a base rate and market multiplier. This independent contractor role offers flexible scheduling, travel support including airfare and hotel, and malpractice coverage for those building trusted relationships to improve member health.
Required Qualifications
