Nurse Practitioner for In-home Visits

Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members by reviewing medication history, assessing medical conditions, and identifying barriers to care. Perform physical evaluations and educate patients on treatment plans within their homes using pre-populated, patient-specific forms. Complete an average of 6-9 visits daily with a base rate of $80 per assessment, earning up to $2,500 to $4,000 weekly with a market multiplier option. Shifts run Monday through Sunday; no nights or weekends required. This role is for board-certified nurse practitioners working as independent contractors on short-term or long-term bases.
