Job Summary
Board-certified Nurse Practitioner contractors needed to conduct in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members. Conduct in-home visits focusing on overall health, identify barriers to care, and perform a physical evaluation in the member's home. Pre-scheduled visits with health plan members; no prescribing, no lab orders, or treatment changes. Independent contractor (1099) with a flexible schedule; shifts available Monday–Sunday. Compensation ranges from $2,500 to $4,000 per week (average 6–9 visits daily with base rate of $80 per completed assessment) with potential increases up to 135% via Market Multiplier and pay-per-test model. Provides malpractice coverage and travel supports (airfare/hotel/rental car) based on eligibility. Requires state license; Medicaid license or enrollment supported by the company.
