Nurse Practitioner for In-home Visits

Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members by reviewing medication history, conducting physical exams, and identifying care barriers. Perform assessments using pre-populated, patient-specific forms without prescribing medicine, ordering lab tests, or altering treatment regimens.
