Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, well-being, and identifying care barriers. Perform medication and medical history reviews alongside physical evaluations in pre-scheduled visits with health plan members. Utilize a provided, patient-specific form for each assessment while reviewing patient understanding of treatment plans. Earn up to $4,000 weekly with an average of 6-9 daily visits, utilizing a pay-per-test model for additional diagnostic and preventive services. Requires a Medicaid license or willingness to enroll and a state license. Independent Contractor status with flexible Monday-Sunday availability.
