Nurse Practitioner for In-home Visits

Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members using provided patient-specific forms without prescribing medicine or altering treatment regimens. Complete an average of 6-9 visits daily to identify care barriers and educate members on their treatment plans. Work as an independent contractor on a short-term or long-term basis with flexible scheduling from Monday through Sunday. Earn up to $4,000 weekly based on completed assessments and pay-per-test models, with additional incentives for availability. Signify Health provides support for Medicaid licensing, malpractice coverage, and travel expenses including airfare, hotel, and rental cars for eligible candidates.
