Job Summary
Conduct in-home visits to perform medication and medical history reviews, physical evaluations, and identify barriers to care for Medicare, Medicaid, and commercial plan members. Complete assessments using pre-populated, patient-specific forms without prescribing medicine, ordering lab tests, or altering existing treatment regimens. Manage 6-9 daily visits with flexible scheduling available Monday through Sunday, earning up to $4,000 weekly based on a base rate of $80 per completed assessment. This role supports Signify Health's mission to build trusted relationships and help people re-engage with their health in a comfortable, one-on-one setting.
Required Qualifications
- Board-certified nurse practitioner
- State License Required
- Independent Contractor, 1099 status
