Job Summary
Board-certified nurse practitioner contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Perform in-home visits focusing on overall health and identifying barriers to care, with a base rate per completed assessment and potential multipliers; no prescribing, no order of labs, or alteration of current treatment plans. Pre-populated, patient-specific forms provided for each evaluation. Flexible schedule with shifts available across the week; healthcare-related travel expenses such as airfare, hotel, and rental car may be reimbursed. State license required; Medicaid license or willingness to enroll strongly preferred. Independent contractor (1099) status. Locations are in-home visits within the specified area; company provides malpractice coverage and additional incentives for availability.
