Job Summary
Conduct in-home visits for Medicare Advantage, Medicaid, and commercial plan members to perform medication and medical history reviews, physical evaluations, and identify care barriers. Complete assessments using pre-populated, patient-specific forms while reviewing overall health and well-being. Earn weekly compensation based on completed assessments and additional pay-per-test models for qualifying diagnostic and preventive services. Work flexible schedules Monday through Sunday without mandatory nights or weekends.
Required Qualifications
- Board-certified nurse practitioner
- State License Required
- Independent Contractor, 1099 status
Desired Qualifications
- Medicaid License or willingness to enroll (with our support) strongly preferred
