Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members to identify care barriers while reviewing medical history and providing education on treatment plans. Complete an average of 6-9 daily visits, earning up to $2,500 to $4,000 weekly based on a base rate of $80 per assessment and a pay-per-test model. Maintain a flexible schedule Monday through Sunday with no night or weekend requirements, operating as an independent contractor with available travel support including airfare and hotel.
Required Qualifications
- Board-certified nurse practitioner
