Job Summary
Contract-based Nurse Practitioner to conduct in-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. In-home visits focus on overall health and identifying barriers to care, pre-scheduled visits in patients' homes, and completion of health plan assessments without prescribing or altering current treatment plans. Independent contractor role with flexible scheduling, opportunities for high weekly earnings (average 6-9 visits daily with base rate $80 per completed assessment and potential increase via Market Multiplier), travel accommodations (airfare, hotel, rental car) as eligible, malpractice coverage, and ongoing incentives.
