Job Summary
Conduct in-person and telehealth patient visits for adults and children, implementing evidence-based clinical protocols and resolving concerns promptly. Collaborate with interdisciplinary teams, including RNs and LCSWs, to coordinate care across home health, hospice, and vendor partners. Maintain accurate documentation of clinical activities and compliance records while adhering to HIPAA and OSHA standards. Participate in interdisciplinary team meetings and disseminate advancements in healthcare practices. Some evening and weekend coverage and on-call rotation for urgent calls are required.
Required Qualifications
- Master's degree in nursing or related science field
- Active and unrestricted NP license
- FNP certification
- 2+ years of direct patient care as a Nurse Practitioner
- Community-based care experience
- Experience with Medicare/Medicaid
- 2+ years of clinical experience treating complex chronic conditions and high-needs patients
- Medicare and LTSS experience
- Willingness to obtain licensure in multiple states
- Excellent verbal, written, presentation, and group facilitation skills
- Ability to clearly articulate and present concepts and models in an accessible manner to Grace at Home's team, investors, partners, and other stakeholders
- Strong critical analytic, thinking and communication skills
- Good judgement, impeccable ethics, and a strong team player
- Desire to succeed and grow in a fast-paced organization
- Travel to patient home is required
- In-office work is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace
- Regular use of the telephone and e-mail for communication is essential
- Sitting for extended periods is common
- Must be able to receive ordinary information and to prepare or inspect documents
- Lifting of up to 10 lbs. occasionally may be required
- Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines
- Good reasoning ability
- Able to understand and utilize management reports, memos, and other documents to conduct business
- Excellent computer skills
- Microsoft applications including Word, Excel, PowerPoint, and Outlook
- Willingness to explore and learn new or unfamiliar applications that may benefit the clinical team

- Collaborate with business partners such as Home Health and Hospice agencies, assisted living facilities, and other vendor partners
- Collaborate with multiple care disciplines such as RN's, LCSW to provide high quality care
- Some evening and weekend coverage for visits and on-call rotation for urgent calls
- 2+ years of clinical experience treating complex chronic conditions and high-needs patients, Medicare and LTSS experience preferred
