Job Summary
Deliver comprehensive preventative care to residents in skilled nursing facilities through on-site visits and remote monitoring. Conduct patient-centered care supported by a dedicated scribe team for efficient documentation. Build strong relationships with facility staff and patients while collaborating with clinical directors to ensure high-quality standards. Participate in quality improvement initiatives to minimize hospitalizations and elevate patient care. Requires an active NP or PA license with one year of geriatric care experience. Hybrid schedule with flexible hours and free CEUs available.
Required Qualifications
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care and documentation review
