Job Summary
Conduct thorough assessments, diagnose acute and chronic conditions, and manage care for senior residents at Applewood Village and Pines Village. Develop, implement, and monitor individualized care plans while prescribing medications and providing health education to residents and families. Collaborate with physicians and nurses to ensure compliance with regulatory policies and participate in quality improvement initiatives. Maintain accurate medical records and work 3 days at Applewood Village and 2 days at Pines Village on a per-diem basis. Requires a Master's degree in Nursing, current NJ license, and 5+ years of primary care experience.
Required Qualifications
- Master's degree in Nursing from an accredited program
- Current Nurse Practitioner license in New Jersey
- Minimum of 5 years of experience in a primary care or geriatrics setting
- Excellent clinical skills and knowledge of geriatric care
- Strong communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Compassionate and patient-centered approach to care
- Work 3 days at Applewood Village and 2 days at Pines Village
Desired Qualifications
- Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) preferred
