Job Summary
Assess patients' medical and mental health needs and provide a plan of care that proactively manages conditions and addresses barriers to care. Ensure compliance with local, state, and federal agencies related to clinical services. Prescribe medications and order lab work, diagnostic procedures, and consultations. Monitor patients' compliance and response to treatment, modifying plans as necessary. Exercise independent judgment to treat acute and chronic conditions while consulting a supervising physician as appropriate. Collaborate with facility partners and health plan leadership to implement Treat in Place Protocols.
Required Qualifications
- APRN or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations and or Fee for Service experience
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
Desired Qualifications
- experience in primary care or internal medicine, preferably with geriatrics focus
