Job Summary
Clinic-based on-site Nurse Practitioner for Family Medicine role delivering telehealth visits to established patients and managing the clinical inbox for care coordination. Flexible scheduling with 4x10s or 5x8s shifts. Works within Banner Health Center in Queen Creek, AZ, collaborating with family medicine, internal medicine, and behavioral health teams to provide coordinated primary care across age groups from pediatrics to geriatrics; role supports integrated system with on-site labs, extended hours, and same-day appointments. DEA with prescriptive authority and FNP designation with AZ license eligibility required; minimum of 1 year of FNP primary care experience; patient-focused, team-oriented clinician. Required Qualifications
