Nurse Practitioner - Family Medicine

Nurse Practitioner (Family Medicine) needed for team-based primary care practice in Locust Grove, GA. Provide outpatient care across pediatric and adult patients, conduct comprehensive health histories and physical assessments for acute, chronic, and preventive conditions, and contribute to a value-based care model with access to integrated care teams and telehealth. Must be graduate and certified from an accredited NP program; hold or be eligible for an unrestricted license; DEA registration or eligibility; Medicare/Medicaid enrollment or eligibility; BLS or ACLS certification; and be comfortable with EMR systems (Athena preferred). 2+ years of outpatient NP experience is preferred but not required. The role supports flexible scheduling and collaboration with clinical leadership and cross-trained care-management teams, with emphasis on delivering high-quality, connected care through an optimized EMR and virtual tools.
