Job Summary
Nurse Practitioner — Family Medicine in a team-based primary care setting in Locust Grove, GA. Focus on providing outpatient care to pediatric and adult patients across acute and chronic health needs within a value-based care framework. Role includes comprehensive health histories and physical assessments, collaboration with cross-trained care management and population health teams (including pharmacy and social workers) to support patients, and delivering care through in-clinic and telehealth modalities with an optimized EMR (Athena) and access to a virtual scribe (Suki). The practice emphasizes flexible scheduling, strong clinical leadership support, and opportunities for 1:1 leadership time, quarterly leadership connects, and clinic town halls to foster clear communication and provider success. Minimum requirements include NP graduation and certification, an active or eligible state license, DEA registration, Medicare/Medicaid enrollment eligibility, and BLS or ACLS certification. Experience with outpatient care and comfort treating both pediatric and adult populations are valued to deliver comprehensive primary care across ages.
Required Qualifications
- Must be a graduate and certified from an accredited Nurse Practitioner program
- Current, unrestricted license in practicing state or be eligible for licensure
- Active DEA registration or be eligible for registration
- Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Two or more years of outpatient clinical experience as a Nurse Practitioner (preferred but not required)
- Proficiency with Electronic Medical Record (EMR) system; Athena preferred
