Job Summary
Nurse Practitioner (Family Medicine) in a team-based primary care practice in Locust Grove, GA. The role supports care across pediatric and adult patients with acute, chronic, and preventive needs, utilizing an optimized EMR with telehealth capability and a virtual scribe, and contributing to population health and care-management initiatives. The NP will collaborate with clinical leaders and embedded care teams, participate in regular leadership and clinic-wide meetings, and deliver comprehensive, patient-centered care in a value-based framework. Requires graduation from an accredited NP program, current license or eligibility, DEA registration or eligibility, Medicare/Medicaid enrollment or eligibility, and BLS or ACLS certification; experience in outpatient settings is preferred. Required Qualifications
- Must be a graduate and certified from an accredited Nurse Practitioner program
- Two or more years of outpatient clinical experience as a Nurse Practitioner, preferred but not required
- Current, unrestricted license in practicing state or be eligible for licensure
- Active Drug Enforcement Agency (DEA) registration or be eligible for registration
- Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Comfortable with Electronic Medical Record (EMR) system; Athena preferred
