Job Summary
Nurse Practitioner (Family Medicine) sought for a team-based primary care practice in Locust Grove, GA. Provide outpatient care to pediatric and adult patients, conducting comprehensive or episodic health histories and physical assessments across acute, chronic, and preventive care. Work within a value-based care framework with access to clinical leaders, cross-trained care management and population health teams, and a telehealth capability including a virtual scribe. Requirements include graduation and certification from an accredited NP program, current unrestricted state license or eligibility, DEA registration or eligibility, Medicare/Medicaid enrollment or eligibility, and BLS/ACLS certification. Proficiency with EMR systems (Athena preferred) is expected. Flexible scheduling and opportunities for professional development and leadership engagement are provided.
Required Qualifications
- Graduate and certified Nurse Practitioner from an accredited program
- Current, unrestricted license in practicing state or eligible for licensure
- Active DEA registration or eligible for registration
- Active Medicare/Medicaid Enrollment or eligible to enroll
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Comfortable with Electronic Medical Record (EMR) system; Athena preferred
