Job Summary
Nurse Practitioner in a team-based primary care practice in Locust Grove, GA. The NP will provide care for pediatric and adult patients within a family medicine/primary care setting, including comprehensive health histories and assessments for acute, chronic, and preventive health. The role supports telehealth capabilities and an optimized EMR workflow, with cross-trained care management and population health teams embedded in the clinics. Ideal candidates have completed an accredited NP program, hold an unrestricted license in-state (or eligibility), DEA registration (or eligibility), Medicare/Medicaid enrollment (or eligibility), and BLS or ACLS certification; experience in outpatient care is preferred. The position emphasizes collaboration with clinical leadership, flexible scheduling, and ongoing professional development within a physician-led, value-based care network.
Required Qualifications
- Must be a graduate and certified from an accredited Nurse Practitioner program
- Two or more years of outpatient clinical experience as a Nurse Practitioner, preferred but not required
- Current, unrestricted license in practicing state or be eligible for licensure
- Active Drug Enforcement Agency (DEA) registration or be eligible for registration
- Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Comfortable with Electronic Medical Record (EMR) system; Athena preferred
