Job Summary
Nurse Practitioner (NP) for family medicine joining a team-based primary care practice in Locust Grove, GA. Responsibilities include treating patients across all ages in traditional, home, and telehealth settings within a value-based care framework; collaborating with cross-trained care management and population health teams (including pharmacists and social workers); utilizing an optimized EMR with telehealth capability and a virtual scribe; participating in flexible scheduling; and engaging with clinical leadership and regular practice meetings to drive provider success. Must be a graduate and certified NP, with a current license or eligibility, DEA registration or eligibility, Medicare/Medicaid enrollment or eligibility, BLS or ACLS certification, and experience or comfort with EMR systems (Athena preferred).
Required Qualifications
- Must be a graduate and certified from an accredited Nurse Practitioner program
- Two or more years of outpatient clinical experience as a Nurse Practitioner, preferred but not required
- Current, unrestricted license in practicing state or be eligible for licensure
- Active Drug Enforcement Agency (DEA) registration or be eligible for registration
- Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Comfortable with Electronic Medical Record (EMR) system; Athena preferred
