Job Summary
Nurse Practitioner to service patients in the Spartanburg area; evaluates high-risk patients, coordinates care plans, and provides care management across homes, facilities, telehealth, clinics, and specialties. Collaborates with an interdisciplinary team to ensure continuity of care, communicates with case managers, clinical coordinators, social workers, specialists, pharmacists, and other team members, and documents care management activities for regulatory compliance. Role involves travel between offices, facilities, and patient homes on a daily basis; offers a full-time salary-based position with a monthly bonus opportunity. Shifts include 8-hour days (Mon-Fri, 8:00am-5:00pm) in service areas such as Woodruff and Wellford, SC. Requires licensing in NP/PA/Physician role with good standing; describes responsibilities across evaluation, care planning, coordination, education, and adherence to HIPAA/Medicare guidelines; emphasizes teamwork, patient-centered care, and adaptability to care settings (homes, facilities, telehealth, clinics, and specialties).
Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
- Must have reliable transportation and be able to travel daily.
- Must be a licensed driver with an automobile that is insured and in good working order.
