Job Summary
Weekend In-House Nurse Practitioner in a Skilled Nursing Facility: provide essential care and leadership within the facility, including admissions evaluations, monthly stability visits, comprehensive physical examinations, prescribing medications, developing patient care policies, coordinating advanced care planning and discharge visits, and ensuring timely documentation. Collaborates with the interdisciplinary team to support physician-patient relationships and maintain high-quality geriatric primary care. No nights or on-call; in-person facility-based role in Oak Lawn, IL.
Required Qualifications
- Active Illinois Nurse Practitioner License (unencumbered)
- Master's Degree in Nursing from an accredited program
- Strong clinical background in primary care and preventive medicine
- Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings
