Job Summary
Weekend In-House Nurse Practitioner to support the primary internal medicine provider within a Skilled Nursing Facility. Lead clinical care for geriatric residents, conduct new admission evaluations and monthly stability visits, perform comprehensive exams, prescribe medications, develop patient care policies, coordinate advanced care planning and referrals, and support physician-patient relationships. Collaboration with interdisciplinary team to ensure high-quality, proactive care with no nights or on-call; IL NP license and MSN required; geriatric primary care experience preferred.
Required Qualifications
- Active Illinois Nurse Practitioner License (unencumbered)
- Master's Degree in Nursing from an accredited program
- Strong clinical background in primary care and preventive medicine
- Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings
