Job Summary
Weekend In-House Nurse Practitioner leading care within a Skilled Nursing Facility, conducting new admission evaluations and monthly stability visits, performing comprehensive physical examinations, prescribing medications, developing patient care policies, coordinating advanced care planning and discharge visits, and ensuring timely communication and documentation. Works collaboratively with physicians and the interdisciplinary team to deliver proactive, high-quality care for geriatric residents; no nights or on-call; focus on internal medicine, primary care, and geriatric care in a SNF setting.
Required Qualifications
- Active Illinois Nurse Practitioner License
- Master's Degree in Nursing
- Strong clinical background in primary care and preventive medicine
- Geriatric or adult medicine experience preferred
