Job Summary
Nurse Practitioner provides direct patient care as hospice attending practitioner, performs medically necessary visits and recertification visits, and participates in coordinating care with the interdisciplinary group; also engages in education, outreach, and leadership activities for the community location. Requires MSN or higher, KY and IN licensure (including DEA), and one year of advanced nursing practice experience; on-call duties and driver’s license required; EMR tools experience preferred.
Required Qualifications
- MSN or higher-level Adult Nurse Practitioner required
- Licensure (or eligibility for licensure) for advanced nursing practice in both KY and IN, including DEA and controlled substance permits, required
- Minimum of one year experience in full time advance nursing practice required
- Primary care, palliative care, hospital-based, or hospice experience preferred
- Fulfill requirements of on-call duty policies
- Must have a valid driver’s license in accordance to policy
- Use of technology-based tools, including electronic medical record, strongly preferred
