Job Summary
Provide comprehensive primary healthcare services to individuals and families, with a specialized focus on refugee health and pediatric care. Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines. Prescribe medications and therapeutic interventions within the NP scope of practice. Manage and coordinate care for patients with chronic and complex health conditions. Collaborate with an inter-professional healthcare team, including physicians, nurses, social workers, and community health professionals. Provide patient education, counseling, and advocacy to promote health and well-being. Maintain accurate and timely electronic medical records and documentation. Participate in quality improvement initiatives and professional development activities. Support workplace equity, diversity, and inclusion. Full-time (37.5 hrs/week) with flexible scheduling available; on-call stipend included.
Required Qualifications
- Successful completion of a post-graduate Primary Care Nurse Practitioner program
- Current Extended Class Registration with the College of Nurses of Ontario
- Proof of current membership or alternative professional liability coverage
- Current CPR certification (Level C or HCP)
- Demonstrated experience in community and/or primary care
- Experience in chronic disease management and prevention
- Strong ability to work both independently and collaboratively within a healthcare team
- Excellent oral and written communication skills in a clinical setting
- Proficiency in the use of electronic medical records (EMR)
