Job Summary
Manage a clinical caseload of ESRD and pre-ESRD patients across correctional, home dialysis, and nursing home settings. Perform comprehensive history-taking, physical assessments, and treatment planning while prescribing medications in accordance with Florida Board of Nursing laws. Monitor dialysis patients in partnership with nephrologists, ensure timely follow-up in Electronic Health Records Systems, and educate patients on disease processes and wellness promotion. Collaborate with multidisciplinary teams to ensure continuity of care, contribute to new service development, and participate in quality improvement initiatives. Work independently under the direction of the Medical Director, maintaining compliance with regulatory and correctional standards. Schedule is three times monthly.
Required Qualifications
- Master's degree in nursing from an accredited school
- Current RN License
- active Nurse Practitioner certificate
