Job Summary
Nurse Practitioner at GuideStar Eldercare provides autonomous, private-practice–style clinical care with administrative support, traveling daily from home to assigned nursing facilities (Kansas City, Topeka, and Lawrence, KS). The role includes initial evaluations, ongoing therapy services, cognitive testing, documentation of service needs, and building positive professional relationships with facilities, patients, and families as part of a multidisciplinary team focused on neurobehavioral dementia and neurocognitive care. Requires NP licensure and ability to function autonomously within the clinical leadership structure, with credentialing and background checks as part of pre-employment screening.
Required Qualifications
- Proof of Nurse Practitioner licensure
- Ability to work as part of an interdisciplinary treatment team
- Good time management skills
- Able to function autonomously with regional Area Manager support
- Pass pre-employment background check and drug screening
- Complete credentialing process
