Job Summary
The Advanced Practice Provider delivers primary and urgent care to nursing-center residents in collaboration with center leadership and other medical providers. The role supports AlignMed Partners’ mission and works with Medical Director/Attending Physician for patient-specific care, providing on-site collaboration and care coordination within two centers in Keene, NH. The position is a full-time float between two centers, open to Nurse Practitioners or Physician Assistants, with a focus on geriatric and adult patient populations and opportunities for mentorship, career advancement, and incentive-based compensation.
Required Qualifications
- Graduate from an accredited School of Nursing program
- Licensed as a registered nurse
- Certified as a Nurse Practitioner by graduating from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing
- Concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP) or equivalent experience
- Physician Assistant path: graduate from ARC-PA accredited program and pass PANCE to use PAC designation
- PA state licensure required
- NCCPA certification required
- Minimum two years experience with adult or geriatric population preferred
- APRNs: unrestricted professional license and DEA (per state of practice) where allowed
- PAs: unrestricted professional license and DEA where practice allows
