Nurse Practitioner

In-home health evaluations for Signify Health (CVS Health). The role supports visiting members in their homes for 45-minute evaluations, including medication and medical history review, physical examination, and potential diagnostic tests (e.g., spirometry, eye exams). The clinician uses an iPad and works within a national network to connect members to appropriate care, traveling locally within licensed states and conducting virtual visits as needed. Primary workplace is the member's home; covers Grand Junction MSA (Colorado) with travel as required. The position is full-time with benefits, and requires active licenses and board certification (NP or PA); multi-state licenses are preferred. Other duties include providing Diagnostic & Preventive Services as indicated and participating in educational activities and CEU allowances.
