Job Summary
Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab clinician within SNF/post-acute settings, functioning with clinical autonomy and collaborating with PCP and rehab therapy teams to improve patient function and quality of life. Lead interdisciplinary care discussions, manage conditions common in SNFs (e.g., CVA/stroke, neurological conditions, postoperative orthopedic care), assess barriers to therapy (pain, spasticity, neurogenic bladder/bowel, etc.), and participate in ongoing education, Didactic lectures, and regional meetings. The role emphasizes post-acute care, geriatric patient focus, autonomous practice, and a pathway toward CRC leadership through training opportunities, credentialing, and continuous professional development.ESSENTIAL QUALITIES include empathy, strong interpersonal skills, adaptability, commitment to high-quality patient care, and ability to collaborate with PT/OT/SLP and internal medicine teams to optimize patient outcomes.
Required Qualifications
- Nurse Practitioner or APRN or Physician Assistant license
- New Mexico License
- Ability to work in the United States
