Job Summary
Conduct comprehensive medical, functional, and psychosocial assessments and independently prescribe, adjust, and discontinue medications including opioids under physician orders. Perform provider face-to-face, end-of-life, and recertification visits while developing and revising plans of care in collaboration with the interdisciplinary group. Provide education and counseling to patients, families, and staff regarding diagnoses, prognosis, and goals of care, and participate in the provider on-call rotation. Maintain accurate documentation in the electronic health record and contribute to quality improvement initiatives. Requires a Doctor of Nursing Practice (DNP) and at least two years of hospice or acute care experience; base pay ranges from $162,968 to $173,243.
Required Qualifications
- Master's Degree Nursing
- Doctor of Nursing Practice (DNP)
- At least two years of experience in hospice, acute care, or transitional care settings
- Active state‐issued NP license
- Active RN license
- California Nurse Practitioner License
- California Nurse Practitioner Furnisher License
- California Registered Nurse License
- National Provider BLS – American Heart Association certification
- Ability to lift and move objects up to 50 pounds
- Close and distance vision
- Color and peripheral vision
- Depth perception
- Ability to adjust focus
- Exposure to bodily fluids, infectious diseases, and conditions typical to the field
- Routine use of standard medical equipment and tools associated with clinical care
Desired Qualifications
- Experience in advanced pain and symptom management
- Experience in end‐of‐life care
- Experience in hospice recertification assessments
- Certified Hospice and Palliative Care Certification (CHPLN)
