Job Summary

Provide assessment, diagnosis and treatment as a Nurse Practitioner in a team-based primary and community health setting in San Jose, CA. Responsibilities include prescribing medications, developing and implementing treatment plans with patients and care team members, performing physicals, reviewing patient histories, and delivering preventive and behavioral health services. Collaborate with subspecialists and integrated care teams, participate in on-call after hours rotation, maintain clinical records per standards, participate in team meetings and clinical quality improvement processes, and provide coverage for other clinicians as needed. Requirements include a Master’s degree or higher in Nursing, California RN or CA-Board license as a Nurse Practitioner or PA, DEA registration, 2+ years of clinical experience, knowledge of family/general internal medicine or pediatrics, familiarity with community health centers, strong communication and teamwork skills, and bilingual ability preferred (Spanish, Mandarin, Cantonese, etc.). The role is in San Jose, CA with on-site duties; FNP preferred; EHR/healthcare-regulatory knowledge and a commitment to inclusive, multidisciplinary care are expected.

Required Qualifications

  • Masters Degree or higher in Nursing
  • Licensed with the California Board of Registered Nursing as a Nurse Practitioner or Licensed with the California Physicians Assistants Board
  • Family Nurse Practitioner (FNP) preferred
  • Current DEA registration
  • Two or more years of clinical work experience
  • Knowledge of the principles and practices of family medicine, general internal medicine, or pediatrics
  • Knowledge of the local, state, and federal regulations pertaining to medicine and to community health centers
  • Knowledge of the structure and function of community health centers
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team
  • Ability to communicate well with people of diverse cultural professional and experiential backgrounds
  • Excellent analytical and critical thinking skills
  • Bilingual in one of the Santa Clara County threshold languages (Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese preferred)
