Job Summary
Conduct comprehensive assessments for patients with chronic conditions like diabetes, hypertension, and COPD, then develop individualized care plans in collaboration with the interdisciplinary team. Monitor patient progress using remote patient monitoring tools, adjust treatment plans, and provide ongoing education while coordinating care across providers and community resources. Utilize telehealth platforms and electronic health records to document care and communicate effectively. Participate in on-call rotation, quality improvement initiatives, and continuing education to maintain professional affiliations and certifications. Ensure compliance with CMS guidelines for billing and document adherence to OSHA safety and universal precaution protocols. Requires an active Virginia Nurse Practitioner license, two years of experience with the frail or elderly, and proficiency in EHR systems.
Required Qualifications
