Job Summary
Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and additional diagnostic tests such as spirometry or diabetic eye exams when ordered. Utilize an iPad and intuitive clinical workflow to perform these services while connecting one-on-one with health plan members in their private residences. Provide all Signify Health services indicated, including Diagnostic and Preventive Services products, without prescribing medicine, ordering lab tests, or altering current treatment regimens. Work as a clinician colleague within a national network, fulfilling requirements for specified roles while remaining flexible to travel locally and conduct virtual visits as needed. This part-time position requires an active, unrestricted license and board certification, with anticipated weekly hours of 29.5. Candidates must be comfortable performing duties in diverse home environments that may contain pets, secondhand smoke, or airborne allergens.

- Able to cover the following primary locations: Bellingham and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
