Job Summary
Provide comprehensive care to seniors following clinical standards while collaborating daily with a multidisciplinary team to ensure coordinated, high-quality outcomes. Manage referrals, hospital/SNF coordination, DME, and home health services, and document care accurately with support from quality-based coders. Participate in on-call rotation and support growth plans at the center. This onsite float role supports our San Antonio area clinics within a collaborative, value-based care model focused on physical, emotional, and social wellness.
Required Qualifications
- Master's Degree in Nursing or completion of a PA program with board certification
- active, unrestricted NP or PA license in the state of practice
- required to be screened for TB
Desired Qualifications
- 2 or more years of experience in value-based care, primary care or Medicare Advantage settings
- active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
- Bilingual in Spanish and English
