Job Summary
Nurse Practitioner in a clinic setting responsible for evaluating high-risk patients, coordinating care plans, and providing care management across clinics, homes, facilities, and telehealth. Collaborates with an interdisciplinary care team to ensure continuity of care, participates in team meetings, and adheres to HIPAA and Medicare guidelines. Offers education and resources to patients and families, supports quality measures, and works flexible shifts (12-hour or 8-hour) with potential monthly bonus opportunities in a full-time salary-based role serving the Chapin area within a physician group delivering primary care, specialty services, and pharmacy support across SC/GA.
Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
- Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
- Must be able to travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
