Job Summary
Conduct comprehensive primary care visits and physical exams in patient homes, managing chronic disease through longitudinal care plans for high-risk individuals. Partner with an interdisciplinary team to ensure seamless care transitions and treat acute needs to prevent ER visits, addressing social determinants of health rather than just symptoms. Maintain a dedicated patient panel with full support while exercising true autonomy in problem-solving. Requires a Master's degree, active NP license, and Board Certification. Salary ranges from $104,763 to $166,750 plus mileage and benefits; relocation is available. No visa sponsorship offered.
Required Qualifications
- Master's Degree (NP)
- Active, unencumbered NP license
- Board Certification (ANCC or AANP)
- Valid driver's license
- vehicle for local field-based travel
