Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence. Participate actively in multidisciplinary team meetings, case conferences, and educational activities. Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems. Provide education and counseling to patients and families regarding the rehabilitation process, expected outcomes, and post-discharge care. Join a collaborative, patient-centered physiatry team focused on restoring function and improving quality of life for individuals with injuries, illnesses, or disabilities.
