Job Summary
The Nurse Practitioner (NP) is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, and home settings. Key responsibilities include performing geriatric assessments, developing nursing plans of care, diagnosing and treating patients, facilitating referrals, and documenting care per established standards. This position requires independent and collaborative work with healthcare professionals and participation in clinical rounds and quality management programs. Candidates should demonstrate strong critical thinking skills, effective communication, and a commitment to patient advocacy.
Required Qualifications
- Must be a licensed Nurse Practitioner
- Ability to function independently as a certified nurse practitioner
- In-depth knowledge of patient population and healthcare protocols
