Job Summary
Deliver comprehensive outpatient care to adult and senior patients while diagnosing and managing moderately to complex medical conditions. Maintain accurate medical records, coordinate referrals with specialists, and participate in clinical quality improvement programs. Exercise independent clinical judgment and collaborate with interdisciplinary teams to ensure holistic care. Work on site at CenterWell Regency in Lexington, KY, reporting to the Associate Medical Director. Complete quarterly limited on-call coverage by phone and maintain proficiency with electronic health records. Requires a Master's degree, active NP license, and two years of outpatient experience.
Required Qualifications
- Master's degree in nursing or completion of a PA program with board certification
- Active, unrestricted NP or PA license in the state of practice
- Must be willing to participate in a quarterly limited on-call coverage by phone
- Proficiency with electronic health records and digital documentation
- 2+ years' experience working as a NP or PA in an Outpatient clinical setting
- Must be screened for TB
Desired Qualifications
- Specialty training in an Outpatient clinical setting in Primary Care, Family Medicine, Internal Medicine, or Geriatrics
- Experience working with senior populations or in Value-Based Care settings
- Proficiency with electronic health records Athena Health and digital documentation
