Job Summary
Provide comprehensive care to seniors following clinical standards while collaborating daily with physicians, pharmacists, and behavioral health specialists to ensure coordinated, high-quality outcomes. Manage referrals, hospital/SNF coordination, DME, and home health services, and document care with support from quality-based coders. Participate in on-call rotation and support growth plans at your center. This in-person clinic-based role requires a Master's degree in Nursing or PA program completion with board certification and an active, unrestricted license. You will report to the Area Medical Director and undergo TB screening. CenterWell Senior Primary Care offers fewer patient visits per day to maximize meaningful time with patients, focusing on physical, emotional, and social wellness within a value-based care model.
Required Qualifications
- Master's Degree in Nursing or completion of a PA program with board certification
- active, unrestricted NP or PA license in the state of practice
- experience providing value-based care
- TB screening
