Job Summary
Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, working onsite at the Conviva West Palm Beach Center. You will provide comprehensive care to seniors following clinical standards, collaborate with a multidisciplinary team, manage referrals and care coordination (hospital/SNF, DME, home health), document care with support from quality-based coders, participate in on-call rotations, and contribute to growth plans at the center. Requires a Master's degree in Nursing or a board-certified PA program, and an active, unrestricted NP/PA license in the state. Preferred experience in value-based primary care or Medicare/Medicaid settings; DEA license and Medicaid/Medicare numbers or ability to obtain; familiarity with HEDIS indicators and Medicare guidelines. Language proficiency in Spanish and English required. Location: Conviva West Palm Beach, FL; Schedule: 40 hours/week; Compensation: $123,800-$170,300 per year with potential bonuses.
Required Qualifications
- Master's Degree in Nursing or completion of a PA program with board certification
- Active, unrestricted NP or PA license in the state of practice
- Ability to be screened for TB
- Spanish and English Language Fluency
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
